BASIC & VOLUNTARY LIFE/AD&D
Protecting your family’s future is no doubt one of your highest priorities. One way to help achieve this goal is through life insurance. In addition to the basic life insurance, you will also get the same amount for accidental death and dismemberment (AD&D) insurance. This benefit offers financial protection to you in case of injury or death caused by an accident.
Life/AD&D Insurance Benefits
Group Number(s): 1148278
- Basic Life/AD&D | Two times your base salary up to a maximum of $600,000.
- Employee Voluntary Life/AD&D | You may purchase amounts in $10,000 increments up to the lesser of $500,000 or 5 times annual salary.
- Spouse Voluntary Life/AD&D | You may purchase amounts in $5,000 increments to a maximum of $250,000 for your spouse. Not to exceed 100% of crew member benefit.
- Child Voluntary Life/AD&D | You may purchase life insurance for your child(ren) up to a maximum of $10,000.
- Guarantee Issue | Employee – $180,000; Spouse – $50,000; Child(ren) – $10,000. You will have guaranteed issue no medical questions asked, only if you enroll during your initial eligibility period.
- Evidence of Insurability | Required to complete health questionnaire if you apply after your initial enrollment, or elect any amount over the guaranteed isseue.
You, your spouse, and children will receive the same amount of Voluntary Accidental Death and Dismemberment insurance, when enrolled in Voluntary Life.

EVIDENCE OF INSURABILITY
The evidence of insurability form is completed when somebody wants to obtain voluntary life insurance limits above the guaranteed issue amount, or when enrolling after initially eligible for coverage.
During your first enrollment, you can enroll up to the guaranteed issue with no medical questions asked.
